Well, the sewer level in Resident Evil 4 was very fun, creepy and great designed (like everything in that game), so it falls not under the category "Bad Dungeon/Sewer" Level.

Not sure if it counts as a level, but the "Fish Tank" room in Fatal Frames 1/3.

Trying to fight a ghost that can teleport and float whilst you're walking on thin, flimsy, narrow planks = not fun

first /big/ boss fight in Max Payne 1 (aw crap i can't remember the dudes name. he's insane and satan worshipping or something...LUPINO!) or the sniper level in CoD4 where you have to carry the support guy around... (i'm STILL stuck at the end of that one)

it ticks me the f*ck off. the one time i actually got to Lupino himself (took me a good 20 minutes), i forgot to save and died in 2 seconds and said "SCREW THIS!" and turned the game off... never felt the urge to start it back up again.

Ugh, I remember Ravenholm, as soon as I saw a Poison Headcrab my arachnophobia kicked in and I immediately turned off the game.
