Actually, I should be able to do it in blender with zero code changes. Let me mess with it.
EDIT: I could, but it would be a mess, you'd have to edit all existing labels, and new ones would need to be made differently, cassate labels would look like ****, a code edit makes more since, I might be able to do it if I knew which file.

I had few time to look at this but dialogs.py is probably the file you're looking for. But maybe there's more to edit
I thought that having to refer to only one copy of the "underneath" png that could be in the mod directory would be nice, because there would be no need for a switch to display or not that underneath when you're (or not) using the cd mod. I was trying to use a copy of the label.dae called "underneath.dae" and try to position it under the CDs but I'm stuck there, I don't really understand yet how to display a new .dae and it's mesh (the label is Mesh_001? I can't import the files in blender right now, I sometimes have collada problems) I'm reading on this.
